Mental health and other chronic illnesses present many challenges for children and adults in the community, along with their family members. Pinnacle Community Services offers a variety of services to aid both children and adults in their recovery efforts. Once enrolled in the program, individuals (and families) will work with a Care Manager who will support and assist them in navigating the sometimes confusing and complicated health care and social service systems to access the services they need to stay healthy and to improve their quality of life.
The heart of our service is building a “supportive relationship” with the people we serve, helping them find the right care and services. A Care Manager can help you navigate, learn about services and get connected to the services you need. There is no cost to the consumer and services are offered on an outreach basis.

Adult Care Management
Mental health and other chronic illnesses impact the lives of many individuals and families in our community. Any Niagara County resident 18 years of age or older, who has been diagnosed with a serious and persistent mental illness or other chronic illness, may qualify for Pinnacle Community Services’ Care Management services. Our Care Managers will help develop an individualized care plan (ICP) just for you to help you stay healthy, out of the emergency room and out of the hospital.
Care Managers will work collaboratively with you and other community service providers to help you navigate, learn about services and get connected to the services you need to live in the community successfully and to reach your personal goals. Services are offered on an outreach basis, and there is no cost to the consumer.
About 1 in 5 adults in the United States will have a mental health condition in any given year, but only 41% of them will receive services. We recognize that taking the first step to seek help can be difficult, but we make it as easy as possible for you to get the help you need.

Children’s Care Management
Children under the age of 18 who have been diagnosed with a serious mental illness, emotional disturbance or other chronic illness may qualify for Children’s Care Management services. Specially trained Children’s Care Managers will help develop an individualized care plan (ICP) with youth and their families to help them get the mental health, medical, specialty care and social services they need to meet their specific challenges. Our Children’s Care Managers work collaboratively with community service providers to assure that the individual’s needs are met to help youth stay healthy, out of the emergency room and out of the hospital.
